Breyer Research Dept Survey Card (Late 1970s-Early 1980s) (sale for charity)

This is an incredible piece of ephemera from Breyer history!  I would guess there may be only a handful of collectors who have a piece like this in their collection.  It is a customer survey card from Breyer's Research Department.  It would have been included in a model's box, to be filled out, folded, taped, and mailed back to the company.  We received three with boxed models, all dating to the period between 1975-early 1980s.  We also received one additional one that couldn't be matched to a model, and this is your one chance to get it!

Near Mint with two spots of very minor wear along with typical aged-paper yellowing.
