Four Seasons (Othello) Treasure Hunt Claim Form

If you've collected one or more of the Othello models from 2010's "Four Seasons" Treasure Hunt, chances are good that you didn't keep one of the claim forms for the prize model.  As time passes, these have become more sought-after pieces of Breyer ephemera.  They're great to have in your collection, important for showing, and can increase your model's value if you ever decide to sell him.
